Overview
We are seeking a compassionate and experienced Registered Nurse to join our Home Health team. The ideal candidate will help us to provide high-quality care to patients in the comfort of their homes, focusing on promoting wellness and independence.
Duties
- Conduct patient assessments and develop individualized care plans
- Educate patients and their families on disease management and self-care techniques
- Document patient progress, treatments, and outcomes accurately
- Eventually may be required to have training sessions with caregivers.
Required Skills:
- Knowledge of internal medicine and trauma medicine practices
- Ability to provide care for patients with behavioral health needs
- Proficiency in conducting physical examinations
