Care Manager Responsibilities:
  • Conduct client assessments & develop personalized care plans
  • Communicate with clients & families for service satisfaction
  • Supervise and support caregivers, ensuring top-quality care
  • Maintain compliance with regulatory and agency requirements
  • Manage and schedule caregiver shifts for clients
  • Screen and interview candidates for caregiver positions
  • Coordinate care with other community services as needed
Care Manager Requirements:
  • Minimum 3 years experience in home care, home health, or related field
  • Excellent communication, organization and leadership skills
  • Reliable transportation, valid driver’s license, background check clearance
  • Compassionate, positive and empathetic demeanor
  • Time management and problem solving skills
  • Possess strong computer skills
  • Maintain high ethical standards, flexibility, and reliability
