Position Summary
The Care Manager oversees home care services by coordinating, implementing, and supervising all aspects of client care. This includes care planning, caregiver supervision, client assessments, and maintaining compliance with state and agency regulations. The ideal candidate will have a strong background in home care management, senior care, and caregiver training, with a passion for building positive relationships with clients and families.

Key Responsibilities

  • Conduct in-home client assessments and create customized care plans.

  • Coordinate caregiver introductions for first shifts, and set expectations for service delivery., introductions, and ongoing training programs.

  • Monitor care delivery through home visits, check-ins, and documentation audits.

  • Maintain accurate and up-to-date client records in care management software.

  • Support caregivers with coaching, performance feedback, and skills development.

  • Communicate regularly with clients and families to ensure satisfaction.

  • Serve as on-call support for urgent client or caregiver needs (approx. 2 weekends/month + occasional weekday evenings).

  • Collaborate with healthcare providers, hospice, and other community resources when needed.

Minimum Qualifications 

  • Experience: Minimum of 3 years in home care, home health, or a related field, including direct experience in caregiver oversight, care planning, and client service delivery.
  • Leadership & Judgment: Proven ability to lead and support care teams, manage performance, and confidently navigate challenging or sensitive conversations with clients, families, and caregivers.
  • Problem Solving & Initiative: Proactive and solutions-focused, with strong critical thinking skills and the ability to anticipate needs, resolve issues quickly, and make sound decisions under pressure.
  • Attention to Detail: Strong organizational skills and accuracy in managing care plans, client records, and scheduling in a dynamic environment.
  • Communication Skills: Excellent verbal and written communication skills; able to communicate with clarity, empathy, and professionalism across diverse audiences, including clients, families, and staff.
  • Client Advocacy: Passionate about supporting clients’ dignity and well-being; skilled in identifying needs and advocating for appropriate care and resources.
  • Supervision & Coaching: Capable of supervising, assessing, and coaching caregivers to maintain high standards of care and ensure alignment with client needs.
  • Technology Proficiency: Comfortable using care management software (e.g., WellSky or similar platforms) and digital tools for scheduling, documentation, and communication.
  • Education: Bachelor’s degree in healthcare, social work, or a related field preferred but not required.
  • Certifications: CPR and First Aid certification preferred.
  • Transportation: Reliable personal transportation, a valid driver’s license, and current auto insurance required.
  • Service delivery and support: Ready to step in to fill shifts during emergencies, staff shortages, or high-demand periods.
  • Flexibility & On-Call Availability: Must be available to handle after-hours phone calls for urgent client or caregiver needs, including on-call responsibilities approximately two weekends per month and periodic weekday evenings. 
  • Compliance: Must pass a criminal background check and consistently maintain a professional and compassionate demeanor.

Schedule & Work Environment

  • Full-time, exempt position

  • Monday–Friday, 7:30 a.m.–4:00 p.m. with flexibility for evenings/on-call needs

  • Local travel to client homes in St. Louis, MO and surrounding areas
