We are seeking a results-driven Care Manager to lead care delivery, manage client relationships, and support business growth. This role is responsible for conducting client assessments, developing and overseeing care plans, supervising caregivers, and ensuring high client satisfaction.
The Care Manager plays a critical role in both operations and growth—building strong relationships with clients and families, supporting onboarding, and identifying opportunities to expand services. You will work closely with the Administrator to ensure smooth operations, compliance, and consistent service quality.
· Conduct client assessments and develop care plans
· Update care plans based on client condition
· Communicate with clients and families
· Handle complaints and ensure satisfaction
· Train caregivers on care delivery and safety
· Supervise caregivers and evaluate performance
· Ensure compliance with care standards and regulations
· Maintain accurate client records and documentation
· Conduct home visits and reassessments
· Coordinate with external providers
· Provide ongoing caregiver training
· Support client education and quality of life
· Build strong relationships with prospective clients and families
· Explain care plans and service value clearly
· Support client decision-making process
· Collaborate with Administrator on client onboarding
· Identify opportunities to expand services
