HomeWell Care Services is looking for a compassionate, organized, and dependable Care Coordinator to join our growing team serving individuals and families across Duval, Nassau, and Clay Counties.

This is a part-time position with real full-time potential as we grow. You will work directly with our Agency Directors in a supportive, fast-moving environment where your work genuinely makes a difference every single day.

What You Will Do

∙ Be the go-to person for caregivers and families, keeping communication clear and care consistent

∙ Manage scheduling and step in to cover shifts in the field when needed

∙ Keep client records and care plans current and accurate in WellSky

∙ Support onboarding, compliance tracking, and day-to-day operations

∙ Flag concerns to our supervising RN and Agency Directors and help find solutions fast

What We Are Looking For

∙ Experience in home care or home health, whether as a caregiver, scheduler, or coordinator

∙ Active HHA or CNA certification

∙ Comfortable stepping into the field when the team needs you

∙ Familiarity with WellSky is a plus but not required

∙ Proficient in Microsoft 365 including Word, Outlook, and OneDrive

∙ A warm, professional presence with strong people skills and genuine compassion

∙ Organized, reliable, and able to juggle multiple priorities without losing focus

∙ Valid Florida driver’s license and reliable transportation

∙ Must pass a Level 2 background screening

Where This Role Goes

This is not a dead-end position. As you grow with us the path leads directly into a full Care Manager role with greater responsibility and compensation to match. We are building something and we want the right people building it with us.
