We’re looking for a compassionate, organized, and relationship-driven Care Manager who thrives in a fast-paced environment and is passionate about helping seniors and families live safely and comfortably at home.

This is more than a coordination role — it’s an opportunity to become a trusted advocate for clients, a leader for caregivers, and a key part of a growing mission-driven team.

What You’ll Do

  • Coordinate and oversee personalized care services for clients in their homes
  • Build meaningful relationships with clients, families, and caregivers
  • Conduct client assessments and develop individualized care plans
  • Match caregivers with clients to ensure the best possible fit
  • Monitor quality of care through regular follow-ups and supervisory visits
  • Ensure compliance with agency standards, documentation requirements, and state regulations
  • Maintain accurate and timely client records
  • Collaborate with office staff to ensure smooth scheduling and service delivery
  • Problem-solve quickly and proactively to support clients and caregivers
  • Help create an outstanding client experience rooted in compassion, dignity, and trust
What We’re Looking For

  • Strong organizational and communication skills
  • Ability to manage multiple priorities with professionalism and empathy
  • Experience in home care, healthcare, senior services, case management, or customer service preferred
  • Leadership mindset with a heart for serving others
  • Detail-oriented and comfortable with documentation and compliance
  • Ability to work independently while thriving as part of a team
  • Positive attitude and strong relationship-building skills
