HomeWell Care Services is newly licensed in Reno, and we're hiring our first Care Manager. You'll build this role with us: the assessments, the care plans, the relationships with families, the standard of what good looks like here. We're small on purpose and we intend to stay careful as we grow.

What makes this different

You won't own the schedule. We keep scheduling as a separate role so your day belongs to clients and caregivers. You'll back up the scheduler when it's needed, but filling shifts won't be your job.

Your caseload will be sized so you can do the work properly. No 60-client panels. If you've been stretched past the point of doing right by people, you'll feel the difference immediately.

Your judgment will actually shape the company. You'll serve as Administrator Designee, and the decisions you make in the first year become how we operate.

What you'll do

  • Meet with prospective clients and families in their homes, and assess what they really need
  • Build the service plan and revise it when things change
  • Make supervisory visits, observe caregivers in the home, coach where it helps
  • Be the person families call when something shifts
  • Keep documentation that holds up to a state survey, and ensure quality and compliance
What you need

  • 2+ years in home care, assisted living, hospice, or case management
  • Experience with assessments and care planning
  • Real comfort with older adults and with families under stress
  • Valid Nevada driver's license, reliable vehicle, current auto insurance
  • High school diploma or equivalent
  • Willingness to share an on-call rotation
Nice to have

  • WellSky Personal Care (formerly ClearCare)
  • VA community care experience — TriWest, HSRM, or VA home care authorizations
  • Existing relationships with Reno-area discharge planners, SNFs, assisted living communities, or hospice
  • Dementia certification (CDP, CADDCT, Teepa Snow)
  • Bilingual Spanish
  • Veteran or military family background
Pay and benefits

  • $58,240–$66,560/year at 40 hours per week
  • Health insurance available after 90 days
  • 10 days paid time off, plus 6 paid holidays
  • Mileage reimbursement for all client visits
  • Monthly cell phone stipend
  • Paid certification — we'll cover dementia training or a care management credential
  • Monday–Friday core schedule with shared on-call
