Responsibilities
- Provide compassionate, personalized in-home care
- Monitor client health to reduce hospital re-admissions
- Create a safe, nurturing home environment
- Encourage social interaction and enrichment activities
- Build trust with clients and their families
- Accurately complete care tasks and documentation
- Follow individualized care plans with professionalism
Qualifications & Skills
- Must pass a South Carolina SLED background check
- Minimum of 6 months experience with seniors or chronically ill individuals
- CNA/HHA/PCA certification preferred
- Current TB test required
- Excellent communication and documentation skills
- Reliable, compassionate, and punctual
- Valid driver’s license, auto insurance, and reliable transportation (if transporting clients)
- Experience with WellSky is a plus
- Must pass a 10-panel drug screen
